What's inside

Pocket script cards
What to say for accusations, "I want to go home," questions about a parent who died, and refusing care. Cut out, keep in your wallet.
Behavior and sundowning log
Two weeks of time, trigger, what happened and what helped, so patterns show up and the doctor sees facts, not impressions.
Doctor visit prep sheet
Changes since the last visit, questions to ask, and a place for the answers, on one page you hand over at check-in.
Medication and contacts sheet
Every medicine, dose, prescriber and pharmacy, plus who to call, for the fridge and the go-bag.
"All about Mom" sheet
Routines, likes, fears and what calms her, for aides, respite workers and hospital staff.
Family meeting agenda and task split
A one-hour agenda and a who-does-what grid, so the work stops landing on one person by default.
Money and paperwork map
Accounts, benefits, insurance, documents and where they live, with the questions to bring to an elder-law attorney.
Home safety walk-through
Room-by-room checklist for falls, wandering, the stove, the car and medications.
Memory care tour checklist
What to ask, what to watch for and what to smell, with a side-by-side comparison for up to three places.
First 30 days plan
Visiting rhythm, what to bring, what to tell staff, and how to answer the go-home calls.
Caregiver check-in
A monthly two-minute check on your own sleep, health and limits, with the numbers to call when the answers are bad.

Who it's for

The person holding everything together: the appointments, the pills, the 2 a.m. phone calls. The sheets are built to be handed off, so a sibling, an aide or a respite worker can step in without a 40-minute phone call first.

Questions

Is this the same as the book?

No. The book explains what's happening and what to do about it. The toolkit is the paperwork: fill-in sheets, checklists and pocket cards. They work well together, and each stands on its own.
